PACE provides daily nursing care as well as additional medical and social services at a fraction of the cost of nursing care. To be eligible, individuals must be age 55 or older and submit to a comprehensive assessment to determine the level of care they require. They must also meet Medicaid financial requirements and be able to live safely within their community. Seniors can find a PACE provider in their area by calling 1-855-435-7223.
