Description of Assured Horizons

With a caring approach that comes straight from the heart, Assured Horizons II is dedicated to serving seniors challenged by Alzheimer's disease, stroke and other difficult medical conditions. Our intimate communities are real homes - not “homelike” - just like the ones our residents have lived in their entire lives. We house no more than six residents at a time and are staffed by at least two daily caregivers. At Assured Horizons, we take great pride in developing the highest professional standards in residential care for the elderly.

Featured Amenities

Our home features well-lit rooms, and spacious common areas with plenty of comfortable seating. Our home has available cable television, window treatments and bathrooms with safety features. Common areas include a living room, as well as well-appointed dining room and full kitchen.

Our courteous staff and caregivers prepare nutritious and balanced meals, three times a day, and can serve options for those with dietary restrictions. Because personally engaging activities are essential to residents’ wellbeing, we match our activities, whether fitness, social or spiritual, to individual interests in a personalized way that larger facilities simply cannot.

Available Services

With our caregivers on-site to handle the routine daily chores of housekeeping, there’s no need to worry about the hassles of home maintenance. Tasks such as laundry or linens are handled by our staff as well, so those with mobility issues can live in comfort.

Providing affordable quality wellness solutions for our residents is at the core of our mission at Assured Horizons II. Security is assured by 24-hour staff, available to assist and respond to daily needs. We proactively assist our residents with activities of daily living, ranging from help with showering, ambulation, toileting, dressing and hygiene.

Special Comment: Assured Horizons maintains high staffing ratios which are essential in the proper care of individuals with Alzheimer’s Disease and related disorders. These high staffing ratios promote individualized attention and opportunities to address residents’ individual needs and personal desires. This is accomplished in each home by having no more than 6 residents and at least 2 caregivers. These staffing ratios also increase the long-term stability of Assured Horizons’ caregivers assisting residents.
