Licensing of Senior Homes in ArizonaIn the state of Arizona, there are several types of facilities that provide care services for adults. Facilities that specifically provide assisted living services, such as bathing, grooming or supervision, for adults 60 years or older are classified as assisted living homes or assisted living centers.

Arizona Revised Statute 36-401 defines an assisted living facility as:

  • a residential care institute that provides or contracts supervisory care services, personal care services or directed care services on a continuous basis.
    • Assisted living centers are assisted living facilities that provide resident rooms or units to 11 or more residents.
    • Assisted living homes provide resident rooms for 10 or fewer residents.

Who licenses Assisted Living Homes and Centers in Arizona?

The Arizona Department of Health Services Bureau of Residential Facilities Licensing is charged with licensing and inspecting assisted living homes and assisted living centers. Inspections are performed as part of an annual licensure survey or when a complaint is lodged against the facility.

  • Arizona Administrative Code (AAC) R9-10-803 (D) (4) requires the facility to clearly post the license and location of the most recent inspection report and resulting plan of correction (if applicable).

What is required to obtain an Assisted Living Home or Center license?

AAC R9-10-105 details all the requirements required for licensing which includes owner information, the proposed scope of services, site plan for the facility and requested licensed capacity.

How do I know an Assisted Living Home or Center is licensed in Arizona?

The Division of Licensing Services provides a Residential Service Providers directory of all the licensed assisted living homes and centers in Arizona. Included in the directory are the addresses, contact information and license number of the facilities.

How frequently are Assisted Living Homes and Centers inspected?

An initial inspection is conducted during the licensing process and inspections are then conducted annually. However, if a facility doesn’t receive any deficiencies during an annual inspection, they will not receive another annual inspection for two years.

Where can I find inspection reports for Assisted Living Homes and Centers?

There is a Residential Facilities Provider online search available where you can search for facilities that have deficiencies. Compliance investigations conducted in the last three years are posted. Not all surveys result in deficiencies, so you need to read each survey to determine what the surveyor found.

How long are Assisted Living Homes and Center required to retain their copy of inspection reports?

While there is no specified length of time that assisted living homes and centers are required to retain their copy of inspection documents, AAC R9-10-803 (D) (4) requires that the facility must post their copy of the most recent inspection and plan of correction (if applicable).

The Bureau of Residential Facilities Licensing retains their copies of the inspection records for three years.

How can I file a complaint against an Assisted Living Home or Center and what is the complaint process?

Complaints may be filed online through a web form. Bureau of Residential Facilities Licensing prioritizes the complaints based upon their severity. If residents are in immediate jeopardy, a surveyor will be at the facility within two days. For other complaints where the health and welfare of residents are not at stake, the complaint may be followed up with a phone call or be addressed during the annual inspection.
