At Brookdale Portage II (formerly Clare Bridge of Portage), our professional staff is specially trained to deal with the unique issues seniors with dementia or Alzheimer's face. A structured daily program is personally created for each resident so that they can experience a life that is full of purpose and meaning. Daily mental and physical exercise ensures that our residents maximize their remaining skills and retain as much independence as possible.
Brookdale Portage II is a member of the Brookdale network. Brookdale has been providing top-notch care to its residents since 1978 and is an industry leader in providing housing and services to America's seniors.
Featured Amenities
Residents at Brookdale Portage II have their choice of private or companion apartments that come complete with telephone and cable hook-ups. Small pets are also welcome to join their owners at our community.
For the enjoyment of our residents, we offer a well-stocked library as well as a sun room and family room where our residents can relax. We even have a beauty salon and barbershop on the premises so that our residents can always look their best.
Available Services
Our professional staff is on site 24 hours a day to assist with grooming, hygiene, mobility, incontinence and medication support. If an emergency arises, a nurse is always on call to help with problems.
Three nutritious meals are served to each resident every day and each meal is tailored to suit individual dietary needs. All meals are prepared by trained chefs and served by professional wait staff. Snacks are available throughout the day as well.

I am a friend or relative of a current/past resident
My mother is living in Brookdale Portage, and it's wonderful. The staff is wonderful, caring, compassionate, and everyone is treated with respect. Theyre very happy with their chefs and I know they custom prepare a lot of their meals for certain people who live there. They have field trips, go out to lunch, arts and crafts, workout activities, music, and someone playing the guitar. They have a nice little calendar with all their daily activities. I've seen nothing but caring in place and I can see that the residents are very well taken care of and have close relationships with the staff.

I visited this facility
Theyre very accommodating and caring at Brookdale Portage. It's a beautiful facility but the biggest drawback was the layout. It just wasn't what was right for my mom because she needs more simplicity. They were gathering for some things in the sun room and they have several different places for activities. They have several different dining areas.
