The leadership team includes our Facility Administrator, Director of Nursing, Medical Director, Director of Social Services, Director of Community Relations, Director of Admissions, and Office Manager. Avalon at Northpointe is the only skilled nursing and rehabilitation center in Spokane with a dedicated on-site physician, and our team of expert nurses provides 24-hour nursing in a healing environment. In addition, we provide individualized physical, occupation and speech therapies through Brighton Rehabilitation's therapists. We have a hospital readmission preventable program called "Keep Me Home". In 2012 Avalon Care Center at Northpointe averaged about 10% below the 2011 National average of 20%, for hospital re-admissions for the same diagnosis/injury. This program helps ensure positive outcome driven care is appropriately accessed. Our comfort care program strives to put patients and their family members at ease nearing end of life by reducing pain and addressing physical, psychological, social and spiritual needs.
