San Gabriel Rehabilitation and Care Center is not your typical nursing home. We understand that those in need of our care services are struggling with many difficulties and we strive to provide a comfortable and homelike environment. We offer full private suites and semi-private suites, each unit coming complete with a flat screen TV, cable and Wi-Fi. Additionally, our warmly decorated common areas, lounges and game area gives residents relaxing areas to socialize.

We also offer an activities program to bring welcome excitement such as bingo, community trips, entertainment and culinary demonstrations. Our expert chefs offer a fine-dining experience serving three meals per day and snacks in a restaurant-style setting, while also offering room service.

Available Services

We customize care plans to each resident's individual needs, coordinating between care givers, nurses and therapists to ensure comprehensive wellness. Among the specialized programs are our pulmonary care unit, would care program, physical, occupational and speech therapy programs.

Our skilled nursing services are available around the clock and admissions are accepted 24/7 and 365 days a year. The types of services that you can expect are assistance with activities of daily living such as bathing, dressing, grooming, mobility help and incontinence care. Additionally, we will provide support with nutrition and dietary issues and offer on-site physician visits.
