Creekside Rehabiliatation & Behavioral Health is a 181-bed skilled nursing facility located in Santa Rosa, California. We offer a variety of services to supply the higher level of physical and mental care a patient needs between the acute hospital and their home. Our team of licensed RNs and CNAs, who are led by the Director of Nursing with ten years' experience, provide the following services: physical, occupational, and speech therapies with state-of-the-art equipment, pulmonary and neurological rehabilitation, wound care, IV therapy, enteral feeding and nutrition, tracheotomy care, diabetic management, a variety of activities and outdoor excursions, general nursing services, and caregiver training.
