Our customized services are provided as medically necessary. When someone enrolls into LifeCircles PACE program they agree to use our network of providers. If someone sees a provider that is not part of the LifeCircles network, or sees a provider that hasn’t been authorized in advance, the individual may be fully liable for payment to that provider. Emergency care does not need a prior authorization.

Our Enrollment Coordinators can help you or a loved one find out of if they are eligible for Long Term Care Medicaid. They can walk beside you as you complete the application process.
