Washington County Nursing Home (WCNH) is a county-owned, not-for-profit home located in rural Akron, CO. They average 27 residents and each resident forms relationships with staff and other residents. WCNH is on a journey to transform the traditional, medical-model nursing home into a person-centered model. WCNH plans to replace the existing building with four, 10-person "Green House" homes within the next two years.
