Horizon Healthcare Services, LLC offers the following services: in home care, companion care, personal care and memory care. We provide skilled and non-skilled services to our clients.

In Home Care: Trust Horizon Healthcare Services to provide professional, safe senior care comparable to services offered in an assisted and senior living facility at an affordable rate in the comfort of your loved one’s home.

Companion & Personal Care: We provide one-on-one personalized care to your loved one to stay home safely and assist with activities of daily living. All care is customized to your loved one as every client is an individual with autonomy.

Memory Care: We provide care to your loved one experiencing memory challenges by providing support for those with dementia and Alzheimer’s using a person-centered approach.
