I visited this facility

Atria Willow Park is very comfortable. Cindy was extremely courteous and answered any questions that I had. It looked like it's very well-maintained, they had a park outside, the rooms are small, but it's adequate for some people. Cindy was very nice, helpful, and very informative. They had a barber shop or a beauty shop, they had a dining room, which was very nice and had tables and chairs. I didn't taste the food but I'm sure they probably had good food, and it was a large room. I saw a studio and a one-bedroom. The lighting was fine but the units were very small, though they're adequate for some people; I'm not sure. The kitchen is really small, there's quite no kitchen really. The facility is very clean and very comfortable.

I visited this facility

The Hamptons At Greenridge is extremely nice and very accommodating. The rooms there were a little bit larger than the other place I toured, and it's a very comfortable place as well. It's very nice and very clean. They also have an exercise room, a little library, a barber shop, and a dining room with tables and chairs. When I was there they had just finished their Easter celebration; it was a very nice and large room.

I visited this facility

Brookdale Tyler East was very clean. The staff members were very good. They met me at the door even though I wasn't scheduled. The dining area looked very clean, and there were lots of staff members helping people eat. It was very supervised. They were very polite. The Brookdale is the only one I would consider but they have a waiting list.

I visited this facility

There was no staff when I visited Rosewood Estates. There were dirty handprints on the doorways, all the decorations in the lobby had dusts on them, and it smelled of urine. The rooms were big like an apartment, which was what I was wanting, but I would never put somebody there because it was so dirty. There was no one there to accommodate me. One of the nurse aides showed me around a little bit, but she didn't know any of the financial thing. The dining room was unkempt. The building needed a lot of repair inside. They need to clean better and they need more staffing.

I am a friend or relative of a current/past resident

My husband is at Brookdale Tyler South. I like the place and the room, but I like to be told upfront when I need to pay something. When we went there, I knew I had to pay the $4,900 which I was prepared to do, but I did not know that I was to pay the extra $1,600, and I don’t know why. Yesterday I got this bill, where they told me that they would split the payment for March and April to $2,450, then they tagged on level of care for April, and I wasn’t told they’re going to do that. It’s like they’re trying to sneak it by me or something, and I don’t appreciate that. What I don’t appreciate is I’ve been doing all of his clothes. When they would do them, they would lose them, and I find other people’s clothes with his. I think the staff is very nice and very friendly, but I don’t think they’re paying enough attention to my husband because when they go to the activity room, they don’t even take him. The nurses that are there do a pretty good job, and seemed to be more patient. The room is very nice and large enough, and the bath is very good, too. All the furniture is mine, and why they’re charging $1,600 for insurance is beyond me.

I visited this facility

Rosewood Estates had excellent facilities and caring staff. It's conveniently located, and the grounds were well maintained. The apartments were large enough and comfortable. We stopped for lunch, and it was excellent. It's an excellent choice.

I am a friend or relative of a current/past resident

My mother moved into Rosewood Estates, she likes it, and she’s settled in. She likes the people she takes her meals with, and it’s good so far. I liked the way it’s designed. It's like a cruise ship, and eating and everything happens in one central area, it doesn’t spill over to the living area. They have different singing groups come in and entertain, and eventually I would like to see her get down and start playing card games with the people. She’s very social and she loves being there. The ladies are nice, the workers are really nice, and the people that run it are fantastic. It felt more homey than the others we looked at.

I visited this facility

I had a very good experience at Rosewood Estates and I had no problems at all. They have friendly staff, the place is clean and they have a lot of extra activities for the seniors to participate in. They serve three meals a day with snacks in between. The dining area is very nice. They have an in-house home health care which is good. The staff is very professional and I had a good impression.

I am/was a resident of this facility

I have moved into Rosewood Estates. We have bean bag baseball and other things for activities. It's an easygoing, laid back community. The staff is very efficient and hardworking.

The monthly rates for nursing home care are low in Texas and particularly in Tyler, where it costs just $4,258. Assisted living is only $450 less expensive per month. Financial Assistance for Assisted Living in Tyler

Medicaid in Texas

The Texas Medicaid program is funded by the federal and state governments to provide low-income residents, including elderly, blind and disabled persons, with access to health care services such as doctor visits, hospital care and prescription drugs, as well as long-term institutional nursing care.

To qualify for Medicaid coverage for long-term care, applicants must be aged 65 and older or disabled and have an annual income below the maximum limits of $24,731 for individuals and $33,482 for couples. Low-income seniors who’ve been approved for Supplemental Security Income benefits automatically qualify for Medicaid coverage.

Applications for Texas Medicaid can be submitted online. For help applying for benefits, call (877) 541-7905 or visit one of the nine Community Partner agencies in the Tyler area.

STAR+PLUS Waiver

Texas Medicaid doesn’t pay for assisted living facility fees, but it does offer the STAR+PLUS waiver that may cover the cost of services received in an assisted living settingApproved seniors receive a personalized managed care plan that covers Medicaid health care services and long-term care supports and services such as:

  • Care coordination
  • Transitional services
  • Personal assistance with activities of daily living
  • Assisted living services
  • Respite care
  • Nursing care
  • Physical therapy
  • Medical supplies and adaptive aides


The STAR+PLUS waiver is available to Medicaid-eligible applicants aged 21 or older who’ve been assessed as needing nursing care for medical reasons and aren’t enrolled in other waiver programs.

Medicaid funding for assisted living isn’t an entitlement in Texas, so there may be a waiting list to receive services through the waiver program. Applications are processed based on the needs of the individual, and those deemed most at risk receive assistance first.

Other Financial Assistance Programs for Assisted Living

Optional State Supplementation

Low-income seniors in Texas who qualify for Supplemental Security Income may be eligible for an additional Optional State Supplementation benefit. To qualify for this extra monthly payment, applicants must live in a Medicaid-funded facility and meet the SSI asset and income requirements.

Applications for OSS benefits can be made online through the Social Security Administration website or by contacting the local Social Security office in Tyler at (877) 319-5707.

Veterans Affairs Aid and Attendance Program

Seniors who’ve served in the military, or their surviving spouse, may be able to receive additional funds from Veterans Affairs through the Aid and Attendance program. If eligible, this extra payment is made on a monthly basis on top of the applicant’s regular VA pension.

To qualify for the A&A program, veterans can’t be receiving a VA disability pension and must meet all of these requirements:

  • Have at least 90 days of active duty with at least one day during wartime
  • Be honorably discharged from service
  • Have an annual income below the program’s limit, which is currently $22,577


Additionally, applicants must meet at least one of the following criteria:

  • Need assistance with daily living activities
  • Be bedridden
  • Reside in a nursing home due to poor physical or mental health
  • Have eyesight limited to a corrected 5/200 visual acuity, or have severe concentric visual field loss

The local senior center is open from 8 a.m.-5 p.m. on weekdays, with the addition of a monthly dance night held at 7 p.m. Classes and activities include crocheting, dominoes, oil painting and line dancing.
