We have been really happy with Autumn Leaves of Rockwall. We chose them because the staff seemed really caring and knew all the people there, and the place was obviously well-taken care of. They have to help my sister with just about everything like they have to help her bathe or eat, not because she cannot eat by herself but because she forgets to eat. They have different room sizes; she is in a studio with a bathroom and a room, and it is private. They have 4 different wings. They have a lot more activities and things to help keep people involved, so they aren’t just sleeping around or watching TV.

My mom just moved in and I have been very happy. She has adjusted quicker than I expected and I believe that is to the love that has been shown to her by the staff. I am sleeping better knowing she is well taken care of.

I chose the 5 stars because I was so amazed this facility smelled like home the smell was fresh and very inviting. I was able to see a few rooms on my tour I was amazed, every room was so clean. The love and tenderness the staff was giving there residents brought tears to my eyes later as I spoke of this home. We opened the door that went to the court yard and the alarm went off and a staff member came running down the hall before Sherrie McBurney could tell her it was us, to me this meant there concern for the residents is their number 1 priority. This is a beautiful, clean home. I was very impressed with my visit Sherrie and the entire staff show so much respect, love and care for each resident and each other as a team. This is a 5 star place it was such a nice treat to see a home so full of love for there residents. Its a wonderful place I highly recommend it

Autumn Leaves Rockwall is conveniently located off of I-30 in the middle of many restaurants, shopping, and grocery stores. It is a single level, secure environment, licensed and providing professional 24 hour care, assistance with medication, nutritious meals - whatever your loved one needs and whenever they need it, the staff is available and willing to help. The building is kept up well on the exterior and the interior is very well maintained. It is always clean without odor and almost every time I have been in the building I have seen the maintenance man cleaning or repairing or upgrading an item. The maintenance team takes pride in the building and it shows. Autumn Leaves has transportation available for the residents. Another positive this facility has over many of the others is every time I go in there are residents in the main living room talking, or entertainment in for them. They seem to be a very active community. Some communities prefer if the residents stay in their rooms. This community likes them to be out of their room, even if they are just sitting looking out the window. A licensed nurse works closely with the resident's family and their medical director to change medication as needed. The licensed nurse oversees medication management ensuring your loved one takes the correct medication at the right time. Residents care plans are reviewed regularly and when necessary are adjusted to allow the residents' ever changing needs to be met. I have noticed when a resident is declining the administrator and nurse will ask the family to come in to discuss options. They are honest when it comes to advising the family of a worsening condition. Also the staff works with several national and local agencies to bring addition help in as needed, such as, hospice and home health. The main issue I have observed with this facility is the high staff turnover. Their nurse just left within the past month, they sometimes have an activity director and community liaison and sometimes not. The staff handles the residents’ needs, from tidying up their room to providing helpful assistance with dressing and bathing. And all staff, from nutrition to the nurse, is trained to work with seniors with Alzheimer’s or dementia. This means the entire team tries to provide the best experience possible for our residents. This patient population is not the easiest to work with at times, the staff has to be and is flexible and patient with the residents. I recommend touring the facility.
