Our regional advocacy for Medicare beneficiaries and caregivers support provides navigation through all the complexities of Non-Medical Home Care, Medicare benefits, Medicaid eligibility, elder law, family case management, Caregiver Support Team, and also a Long-Distance Caregivers Team. We are a Medicare provider for non-medical services in the home, health home care, community medical care homes, diabetes care management and hospice care with dignity and compassion working together to serve our seniors and caregivers.
We strive to keep patients the ability to live safely in the comfort of their homes – even with complex medical situations – we evaluate every aspect of our clients' health — medical, functional, psychosocial, emotional and spiritual.
