Diagnosis based care Plan:
After complimentary assessment is done by our staff RN a diagnosis based custom care plan is formulated, which is designed to provide individualized care. In-home care plays a significant role in minimizing hospital readmissions and in preventing complications. During in home assessment we conduct home safety checklist with recommendations and fall assessment questionnaire to ensure the safety of your loved one.
GPS Timecard/ Built in Telephony
At Heritage giving you and your family peace of mind is our priority. For this purpose we have incorporated in our system the industry’s only mobile device time card, which allows us to verify our caregivers’ locations through GPS tracking as they log in to ensure their timely arrival at assigned client’s home. We will get an alert notification if a caregiver is late for 15 minutes. This helps us to make sure our clients have care at all times. It also helps to build trust, accountability and dependability with our clients.
Family Cares online Portal:
This state of the art system allows families to review shift times, clock ins, activities performed, care plans, critical alerts, leave notes for the case manager and download these reports for long term care insurance reimbursements from anywhere at any time. We believe in transparency and information should be available to our clients and their families. Care recipient or families can also invite doctors, external case managers, therapists, dietitians, trust officers, long term insurance companies to interact in the care management of our shared client.
