Heritage Plaza Nursing and Rehabilitation Center offers 24/7 skilled nursing care including: comprehensive wound care, pulmonary management, IV/enteral/parenteral therapies, restorative nursing care, oxygen therapy, social services, medication administration and assistance. Our short-stay rehab offers physical therapy, occupational therapy and speech therapy. We provide hospice care, respite care and long-term care. Our LifeWorks wellness program includes a curriculum of purpose-driven enrichment exercises for the mind and the body. Our Dining Your Way meal service features menu choices and a restaurant-style dining experience for which we also offer personal assistance if needed. We also offer pharmacy and laboratory services as well as podiatry, dentistry and optometry services. We have an on-site beauty/barber salon as well. We accept private pay, Medicare, Medicaid, hospice, respite and participating insurance and managed care programs.
