    My parents were in this facility, one in 'Memory Care' and one in 'Assisted Living'. They had been there since 2010 (beginning in 'Independent Living') and we had expected to have them there for life. Memory Care had deteriorated over the years (bed sores etc..)but the last straw came when our Alzheimer's patient was 'dropped off' alone at the hospital emergency room with no documentation of his meds or insurance. (This was after staff delayed in calling an ambulance for 4 hours once internal bleeding began; they couldn't locate a supervisor so they could not/did not call an ambulance.) There is only one good employee that I know of in this facility: Madeline who is over Assisted Living (she really cares). We moved our Memory Care patient to Autumn Leaves Memorial City(highly recommend) and his wife is close by at Tarrytowne Estates (highly recommend). NO NOT put anyone you care about at the Forum!!!! Most employees don't care; management all the way to the highest level are all inept, tone deaf and just don't care. Food is just awful to the point of inedible; not much going right in this place.

    The employees here do not seem to really care for what they do. It's like they're physically there, but their minds are somewhere else. I had been previously and it didn't seem as bad as this particular time. The place is not kept up very well at all. I saw residents sitting bored, they said activities weren't what they use to be. This place was once a very upbeat environment, but now its going downhill.

    The Forum at Memorial Woods has undergone management changes so the level of quality and care is not as evident. Some of the staff seemed disengaged and not really connected with the residents. While visiting I really couldn't see any direct leadership, it was like everyone was just managing themselves. Overall the facility is not updated, the food is moderate, the living space is adequate, and the staff seems preoccupied with their personal tasks instead of their professional ones.

    the facility is very old and dirty, quality of care is so bad, and food is very bad,no activities to do and the staff just texts on there cell phones all day
