Sadly this facility serves all pre-fab food versus fresh, facility cooked, meals. Snacks are not made in-house but are also pre-fab like packaged cookies, crackers and cheese bits, etc. The caregivers are so wonderful, but the management leaves much to be desired because of the burdens put on caregivers, all the cost-cutting measures taken although the facility is expensive. Not many social activities by professionals are planned--it is all volunteer or the responsibility of the caregivers to do even though they should be doing other work. The facility is clean enough, but definitely some detailed cleaning would also be helpful on a daily basis.

I remember the first time walking into Autumn Grove in search of a "home" for Dad after his stroke. There was no reason to look any further. AG gave me a true sense of calm and relief knowing Dad would be in excellent hands. They are a family. The staff made for an easy transition. They truly do "care" for their patients. Not just in the physical way, but tlc included. Treating everyone with respect and kindness. They care, taking pride in the residents appearance, always being sure they are bathed and dressed appropriately. They allow them to maintain their dignity. In getting to know residents, they are aware of changes in mood, appetite, health and do all things possible to keep them comfortable, safe an happy. The staff encourage residents to participate in activities, treat and talk to them as adults or peers. Their support and love thru Dad's two years was nonstop. Now Mom is there. I feel blessed as I know she is safe, well cared for and even loved. I'm proud to consider myself part of the Autumn Grove family!

After placing our Mother, who has Alzheimer's, in three other dementia/memory care facilities, we found Autumn Grove to be the perfect environment for our Mother to thrive. With a 14 resident maximum, the Autumn Grove staff provides the needed attention to keep the residents safe and give the families peace of mind. I can honestly say that the caregivers treat the residents with love and respect. It's more than just a job to them. The layout and openness of the facility shows off the cleanliness and provides an atmosphere similar to one's own home. The meals are prepared in an open kitchen, so no need to worry about meals prepared in mass quantity out of view. And, the food is always very good! Autumn Grove has been our salvation after several very traumatic experiences in large facilities and personal care homes. It truly has been a blessing to have our Mother cared for at this facility.

My mother spent the final 3 years of her life at Autumn Grove. Suffering from dementia, she was a challenge at times and the caregivers always handled her with grace and love. My mother often times thought she was either on a cruise or at the country club. She loved the food, the decor and the "help" as she called the caregivers. My mom was a very private lady who enjoyed solitude and oftentimes had a few choice words for caregivers and other residents when they interrupted her solitude. She was always treated with respect and the caregivers enjoyed and encouraged her individuality. My mom loved jewelry and all things sparkly. The caregivers threw a princess birthday party for her when she turned 86. As she transitioned to the final stage of life, I couldn't have asked for a more loving or caring or knowledgeable staff to help us through to the end. I cannot say enough praise for the manager and her staff and I thank God that I found Autumn Grove for my mother.

My grandmother received loving care from these wonderful people. We felt like a family with this small community. My grandmother liked the small quiet atmosphere not so loud like other communities. It was the best fit for her. We still visit with caregivers and the other residents from time to time. I wish more families knew of Autumngrove Cottage.

Everyone at Autumn Grove Cottage Champions loves their residents and it shows! The care our mother received was wonderfully respectful, compassionate, and the caregivers were mindful of Mom's personality, likes and dislikes. There was never any worry if we were unable to visit during the week or weekend. Our mother received the same loving care whether we were there or not. We cannot say enough about the loving, caring atmosphere!

We knew finding the right place for Mom would be a challenge, as we had a limited amount of time to look, and wanted the best for her. Autumn Grove was our first stop and we pretty much had our minds made up immediately, although we continued to look in order to do our due diligence. The cottage is well laid out and impeccably clean. A lot of thought was put into making the setup beneficial to the residents in every way. The most impressive part however is the staff. They all seemed cheerful and friendly and it is apparent that they truly care for the residents. Autumn Grove has brought my sister and I peace of mind.

I will always know that my mother had the best people to surround her and give her kind attention and take care of her every need. I visit often each week and reconfirm that she is where I want her to be in the hands of AGC at Champions.

Choosing the right assisted living facility is never an easy task. Senior residents in assisted and independent living facilities should feel comfortable in their choice of locating a home that fits their needs and is inviting to them. When locating a facility for a person who is fairly independent, I find it necessary to match the personality of the facility with the personality of the potential resident or family member. Autumn Grove Cottage in Champions is a family style home, located in a great location. The facility is in walking distance to a major shopping center. Although the area is somewhat busy, there are many entertainment venues within a five mile radius. The family style property is designed to feel like home. Safety measures are in place like guardrails and safe entry ways to facilitate the safety of all residents. Many of the residents have memory care needs. This facility provides above average care plans to assist those patients. With a very small number of residents, staff members are able to provide individualized attention. This is not a busy facility. Rather, it is a facility that provides basic care for seniors who need minimal to moderate assistance with activities of daily living, medication reminders, and memory care. Therapeutic measures are implemented throughout this facility to provide for the cognitive and physical needs of every resident. Residents are given a wide range of options for menus, and menus are amended regularly to assure that the dietary and personal preferences of the resident are adhered to. Activities are also present here and tailored to meet the individual needs and preferences of the resident. The assets of this facility are the small size, therapeutic memory care, and adherence to meal preferences and need. The facility fits well with a person who enjoys small, peaceful environments, and not a lot of people moving about. Autumn Grove Cottage in Champions has a home presence that is calm and loving. Individualized attention makes this home a first choice home for any senior who is not looking for extravagance and style, but for warmth, specialized care, and a peaceful atmosphere. This facility is also moderately priced.

Description of AutumnGrove Cottage Champions

AutumnGrove Cottage is a sixteen resident, private-pay, assisted living home specializing in care for senior adults with Alzheimer’s disease and other dementia-related illnesses. To use an analogy, AutumnGrove Cottage™ is to the assisted living industry what a bed and breakfast is to the hotel industry. It is smaller, homelike in design and appearance, and personal. With only sixteen residents, the staff and families get to know each other in a very personal and intimate way.

There are many aspects that make AutumnGrove Cottage™ a unique concept, but the two most important differences are design and focus.

Design – From the moment you drive into the driveway, our cottage looks and feels like someone’s private home. This is one of the cornerstones in the AutumnGrove Cottage™ belief that people should be able to age in the same manner they are accustomed to living.

Focus – Alzheimer’s and memory care is not something we do it’s ALL we do. Our caregivers are “universal workers”. They are responsible for every need of the resident throughout their day including grooming, bathing, activities, preparing and assisting with meals. Our caregivers get to know the residents’ preferences and provide more personalized care. Our staff-to-resident ratio is over two times higher than most traditional assisted living facilities.
