I Visited This Facility

Oakwood Memory & Senior Care was an old apartment building that they renovated into a memory care facility. It's very crowded with a lot of people, and I don't think the number of staff was enough to take care of the residents. The environment was not very good, and they're right next to a busy shopping center. I don't like it.

I Visited This Community

Oakwood Memory Care Living is listed as an "assisted living" community, yet its primary focus is Dementia and Alzheimer's care. They are more of a board and care facility, than an assisted living one. They offer long term care, short term care, respite care, hospital recovery care and hospice care. They also offer medication management and incontinence care. Because they are a primarily memory care facility, social activities take place in-house that include group activities, games and memory stimulation activities. They offer low sodium and low sugar diets, should a resident have special dietary needs. They were not able to offer me a rate sheet detailing their daily/monthly charges, so I cannot review them about their pricing. I would have to call the manager or owner to obtain that information. The community is very dated and not in the greatest of neighborhoods of Vallejo, but the staff seemed engaged with the residents and generally concerned for their well-being.

Description of Oakwood Memory & Senior Care

Oakwood Memory & Senior Care is committed to providing the best possible care in a home like setting at a very reasonable price. We have private and semi-private rooms available, all with private bathrooms. All rooms have access to one of four private courtyards. We are licensed for ambulatory and non-ambulatory residents and we have a dementia waiver so we can serve the needs of residents with early to mid-stage dementia.

When one chooses Oakwood as their new residence, the first thing we do is thoroughly evaluate care needs. We discuss a care plan that will suit a new resident's needs. For many residents, the plan includes a schedule of daily assisted living tasks for their caregivers. We regularly monitor our care plans, increasing or decreasing assistance as appropriate. Because family members are vital to our residents care and well-being, we also hold care conferences to keep family members well informed about their loved one's changing needs.
