This community made my friends mom feel at home. It has a warm and inviting entrance similar to what she would had at her home. Everyone seemed to be happy and clean. The move made my friend regain her life and be more than a caregiver and made her feel at peace knowing her mom was well cared for.
Staff works very hard to accommodate all levels of dementia. They constantly try to engage (successfully mostly!)my father in activities which helps tremendously. The food satisfies my Dad and they do a great job of engaging family members in activities. They seem to be very busy all the time, which is great for residents, but means that families sometimes need to catch them on the fly. I have great confidence about their oversight of my father's care.
This is a small and intimate facility which offers excellent care for the residents. It is new and well appointed and the staff is very caring and professional. The residents have many activities that they are encouraged to participate in and entertainment to enjoy. The meals are well planned and prepared. Family members are welcome anytime and there are many social activities that family members are encouraged to attend. This is a caring and safe environment that sets one's mind at ease as to the quality of life your family member is experiencing daily.
This facility offers their residents a safe environment where they can be happy and healthy with a staff that is friendly, knowledgeable and helpful. They have two large secure courtyards on either side of the building so that the residents can enjoy the outdoors. The public sitting areas, library and mini diner are very comfortable and inviting to the resident as well as those who may be visiting. The meals that are provided are so good and are designed so that each individual person can enjoy what they are eating whether it is providing finger foods for those who have difficulties using utensils to contrasting colors so that they can see what they are eating. They offer a variety of activities to help the residents stay engaged. The resident rooms are spacious and comfortable with comfortable bathrooms.
This is wonderful facility if you are looking for a nice place for a family member with dementia. Their staff is some of the best trained and are always cheerful and ready to help. They offer customized menus for individuals if needed but the daily menus are made fresh and from scratch and are so good. They are always working on new recipes. The building and grounds are very nice as are the resident suites. Everything is for the residents comfort and security. It's nice that they offer Senior Day Stay for individuals who just need once in a while care. They have a personal laundry service and a beauty salon. This is a wonderful facility for memory care patients. They provide activities to keep residents occupied.
I chose the 5 stars because I was so amazed this facility smelled like home the smell was fresh and very inviting. I was able to see a few rooms on my tour I was amazed, every room was so clean. The love and tenderness the staff was giving there residents brought tears to my eyes later as I spoke of this home. We opened the door that went to the court yard and the alarm went off and a staff member came running down the hall before Sherrie McBurney could tell her it was us, to me this meant there concern for the residents is their number 1 priority. This is a beautiful, clean home. I was very impressed with my visit Sherrie and the entire staff show so much respect, love and care for each resident and each other as a team. This is a 5 star place it was such a nice treat to see a home so full of love for there residents. Its a wonderful place I highly recommend it
This property is tucked away in the Cinco Ranch community near the major toll way and freeway. This is a beautiful facility built specifically for the residents living with memory afflicting diagnosis. The community is strategically set up to accommodate the resident by having themed halls and flowing around the courtyard or enclosed patio. The continual flow of the connecting hallways allow for the uninterrupted travel for residents who need to walk but will not wonder off. The welcoming décor brings a feeling of a cozy home setting. This is a great choice for families looking for a secure and home-like setting for their loved one. The staff is friendly and professional with the residents and visitors as I toured the community.
This picture doesn't truly show how nice this place looks. This facility is very appealing to the eye. When you walk in the faculty members are very friendly, they truly make you feel welcomed. The facility is very nicely decorated on the inside. The food had the place smelling very good I must say. Too bad I didn't have enough time to taste it. I'm sure it would've been delightful... the residents seemed to enjoy it. I would totally recommend this residence.
When touring Autumn Leaves Cityview the staff were friendly and observant. They were aware of the individual needs of the residents and seemed genuinely concerned with their care. The staff are resourceful and encouraging. The residents were happy and enjoying their freedom in a safe environment. The building is clean and the residents appear to enjoy the courtyard. The activities were creative and perfect for memory care residents. The food is accommodating and available for residents with challenges.
Autumn Leaves of Fossil Creek is a beautiful community with a charming courtyard. The building is clean and well maintained. The staff are professional and excited about their community. The activities are stimulating and a great fit for the individual needs of all the residents. This community is resourceful and supportive to the families. The care is average but the staff seem aware of the individual needs of the residents. The menu is accommodating and varied. The staff are friendly and helpful during meal times.
Autumn Leaves West Houston give above average care for their residents. The staff is so supportive and understanding. They help the families transition through the different stages of dementia. Therefore, family members know they have made an excellent choice in choosing their facility. The activities are stimulating for the brain and, suitable for the residents conditions. The layout is clean, spacious and home like. The food is great and healthy.
This is a great facility for social interaction. When I visited Autumn Leaves of Riverstone, I saw many of the residents gathered outside near the courtyard area conversing with one another. I also saw other residents sitting in a lounging area watching people pass by and talking with one another. This facility is well-designed with multi-living areas to foster communication, and this place is perfect for the social senior who enjoys meeting new people.
There are activities here, but nothing grand. Most of the activities appear to be on site. However, I was informed by a staff member that they occasionally schedule off site events. There was a craft class going on when I visited, and I saw a couple of residents preparing to attend. There were also two women sitting in the media room reading and socializing. The facility has a home-like feel, the residents move about from place to place like they would in their traditional home. There is a family-style atmosphere that encourages new friendships and the development of an extended family.
The staff were friendly and helpful. A few of them seemed very motivated and happy, a few others appeared to be worn out. Nonetheless, my overall impression of the staff was good. I even had someone assist me to the dining facility. The meals are great. They are prepared on site, and I only had a pastry the day that I visited. However, they were cooking lunch and the smell was overwhelmingly delicious.
Although this facility is clean, well-functional, and promotes socialization, it may not be the place for every senior. Even though some of the amenities here are upscale, the atmosphere is not one of extravagance. It is a family-friendly facility that is well developed for its purpose. Most services needed are provided on site, and residents have the opportunity to rest and escape the busyness of life.
For seniors requiring memory/dementia care, this is a great place. The staff educates the residents and the family members requiring this care about the process and care plan for memory therapy. I recommend this facility for seniors needing moderate memory care.
I toured this beautiful facility and was amazed at the attention to detail regarding the residents and the design of the facility. The building is made to go in a circle to allow the residence to feel secure and each hall has a theme with well lite halls and well trained staff to ensure each resident is receives the care they deserve. I have visited this community on several occasions and find at each time the staff are very engaging, welcoming, and attentive to the residents as well as the visitors. There are activities daily with the residents. I observed word search, memory building games, music and dancing, exercise, piano playing, etc... There are partners within the community who participate with offering support groups for the families and caregivers to assist with the care of each resident to ensure a healthy way of life. The staff are professional and really appear to care greatly for each resident. When you enter the community you feel like you have entered a well kept home and you feel comfortable enough to stay and visit for a while. The hall ways are themed to assist the residents to find their way to their rooms, to the community living rooms, the linen tablecloth dining rooms, or wherever they may want to go in their community.
Autumn Leaves Rockwall is conveniently located off of I-30 in the middle of many restaurants, shopping, and grocery stores. It is a single level, secure environment, licensed and providing professional 24 hour care, assistance with medication, nutritious meals - whatever your loved one needs and whenever they need it, the staff is available and willing to help. The building is kept up well on the exterior and the interior is very well maintained. It is always clean without odor and almost every time I have been in the building I have seen the maintenance man cleaning or repairing or upgrading an item. The maintenance team takes pride in the building and it shows. Autumn Leaves has transportation available for the residents. Another positive this facility has over many of the others is every time I go in there are residents in the main living room talking, or entertainment in for them. They seem to be a very active community. Some communities prefer if the residents stay in their rooms. This community likes them to be out of their room, even if they are just sitting looking out the window. A licensed nurse works closely with the resident's family and their medical director to change medication as needed. The licensed nurse oversees medication management ensuring your loved one takes the correct medication at the right time. Residents care plans are reviewed regularly and when necessary are adjusted to allow the residents' ever changing needs to be met. I have noticed when a resident is declining the administrator and nurse will ask the family to come in to discuss options. They are honest when it comes to advising the family of a worsening condition. Also the staff works with several national and local agencies to bring addition help in as needed, such as, hospice and home health. The main issue I have observed with this facility is the high staff turnover. Their nurse just left within the past month, they sometimes have an activity director and community liaison and sometimes not. The staff handles the residents’ needs, from tidying up their room to providing helpful assistance with dressing and bathing. And all staff, from nutrition to the nurse, is trained to work with seniors with Alzheimer’s or dementia. This means the entire team tries to provide the best experience possible for our residents. This patient population is not the easiest to work with at times, the staff has to be and is flexible and patient with the residents. I recommend touring the facility.
Autumn Leaves Riverstone is nestled within a very safe and thriving city. Throughout the facility it is clean and kept up neatly. The staff is caring and nice. Their comprehensive of alzheimer allow them to show a lot of compassion towards the residents. They encourage family involvement regularly. During shift change the caregivers are exhausted but continue to keep smiles on their faces. The residents look nice and neat. And they seem to enjoy the mealtime and activities.
Autumn Leaves of Carrollton is a charming community with a relaxing secured courtyard. The staff are focused on supporting the individual needs of the residents by meeting them where they currently are in regards to memory and overall care. The community is very neat and cozy. The building is well maintained and the staff place the care of the residents first. The dinning experience is wonderful because the food is arranged according the needs of the residents. Staff are encouraging during meals and residents are excited about socializing over meals.
Wow, very beautiful on the inside and out! Some of the staff seemed disengaged and not really connected with the residents. This facility would be a good choice if you are looking for a staff with general knowledge, clean living areas, and caring management. Overall the facility is modern, clean, well planned menus, and they have moderate activities for their residents.
I am so impressed with Autumn Leaves Arlington. It feels just like home throughout by feeling warm, peaceful and inviting. The staff is professional and caring. I think the love and cleverness while caring for the residents is what sets this community apart from others. The food smelled and taste okay. Activities are exceptional and very effective. The activities are planned around the personality of the seniors with great response. The residents seem pleased and alert.
We needed to find a place for my mother in law and having been down this road before, knew it would be a daunting task and we would have to make certain concessions in our expectations. Not so, it turns out, at Autumn Leaves. It's impossible to express just how amazed and thrilled we are with this place. The staff are over the top!!! Is it possible that so many other places suffer with sub par staff because all the exceptional ones are here??? I'm thinking so. This staff is something special. We have been trying to put our finger on exactly what it is but it just seems like everyone is genuinely happy to be working there and knows the importance of their role no matter what it is. We are so happy to have found this place and only wish we had made this move sooner. Cannot say enough about how lovely this place is.
Autumn Leaves of Flower Mound is a charming memory care community. When entering the community the lobby is clean well maintained and classy. The residents were relaxed happy and able to be free! The activities are endless and they are perfectly design to meet the needs of individuals with memory loss. The staff are happy and they know the needs and behaviors of their residents. The staff have good customer service and the residents are at home. The rooms are cozy, updated and clean. The menu is varied and accommodating.
I visited this facility, and I was very impressed with the staff. The staff members were friendly and helpful. There were a few staff members who appeared to be distracted when I encountered them, but most were smiling, friendly, and made me feel like I was a valued prospective customer.
The facility provides healthy meals. They are very family friendly, and family members can visit and eat with their loved ones. There were activities planned for residents on and off site that keep the residents active and social. My family member enjoys having her hair styled. This facility has visiting stylists who come to style residents' hair if needed. You do have to pay extra for this service, but it is a great treat once in a while.
The price was moderate in comparison to similar properties, but I felt like it was worth it. I would recommend this facility to any family looking for a place that have rehabilitation services and activities that are cognitively stimulating. I would like to see more activities that residents can engage in that increase physical endurance as well as activities that foster creativity.

The property is in the top tier of the Houston area senior assisted living facilities. The staff provides all the resources you need to make an informed decision upfront, so there are no surprises. They also explain anything that may be confusing or unclear.
Autumn Leaves of Meyerland has wonderful residents that are friendly and sociable. They made my loved one feel at home from the first day that she visited. The staff was willing to provide a tour, and they welcome any suggestions.
